HSE Renew or Review Medical Card

Medical cards and GP visit cards are issued for a period of time. Your card has a ‘Valid to’ date on it.

** We will contact you when we need to review your eligibility. We review your eligibility before we can renew your card. We will write to you by email or post to complete a medical card review. It is important we have your current address.

** If you are no longer eligible for a medical card, we will assess you for a GP visit card. We will write to you to let you know the outcome.

** If you received a letter and PIN from us, inviting you to use the online medical card application system, select ‘Review my medical card’.

National Medical Card Online :
Click on login button.

Enter Email Address
Enter Password
Click on Login button.

Forgotten / Reset Password :
If you can no longer remember your password please enter the email address you used when registering. We will send you an email with instructions on how to reset and choose a new password. There must be an email address set up for your account for this function to work.

2. Click on reset password

Create Account :
Click on Create account tab in login page
Your email address will be your username.
Please enter a valid email address as we will send the account activation link to this email address.

Your account has now been created. You will get an email shortly with instructions on how to set your password. Please ensure you check your spam/junk mail for this email.

Change Your Details

** It is important to keep your details up to date.
** We can then keep in contact with you about your card.
** Let us know if your circumstances change, or if you want to change GP.

Change of address :
Let us know about a change of address through one of the following :
** Change your address information online.
** LoCall 1890 252 919.
** Email clientregistration AT hse.ie.
** You will need to supply a copy of a utility bill, or similar, as proof of address.

Change your GP :
** Download and fill out the Change of GP form in English or Irish when you have changed GP.

Change in circumstances :
** A change in circumstance can affect your entitlement to a medical card or GP visit card.

These include :
** Diagnosis of a serious medical condition.
** Recently becoming married, separated, or divorced.
** Recently becoming widowed.
** Your child/children no longer live with you.
** You have a child under 6.
** You are now over 70.
